Required Documents

Your full name must be on any proof of income documents.

• Identification for all adults in the household:
  1. State Issued ID card
• Proof of Hardship (Must Show Hardship Due to COVID-19)
  1. Unemployment letter; OR
  2. Furloughed letter; OR
  3. Check stubs noting decrease of hours or wages; OR
  4. Letter from employer showing reduction in hours/wages due to COVID-19
• Proof of Income
  1. Check stubs (past 30 days); OR
  2. Supplemental Nutrition Assistance Program (SNAP) benefit letter; OR
  3. TANF benefit letter (Temporary Assistance for Needy Families); OR
  4. Social Security Income (SSI)/Social Security Disability Insurance (SSDI) award letters; OR
  5. Pay history from employer (layoff/reduction of hours); OR
  6. Proof of unemployment payments
• Proof of Household Size
  1. Tax return with all household members listed; OR
  2. Social Security cards for all household members
• Lease Agreement or Mortgage Statement
  1. Copy of lease agreement signed by all parties; OR
  2. Mortgage statement; OR
  3. Notice of delinquency from landlord or mortgage company; AND 
  4. Landlord acceptance of terms of assistance
• Utilities
  1. Valid electric, gas, phone services, or internet bill 
• Any other documents requested to determine eligibility
